To begin let me greet all of you and welcome you to this presentation on the basic tenets of care coordination. There is coordination of care which is a crucial task that entails the management of patients’ services and information by all stakeholders involved in the care of a particular patient to enhance the safety of the patient and improve the outcomes of care.
